Prompt Page and Firefox

We have a prompt page that is using the date prompt. When the user runs the report the checkbox next to the date calendar do not exist in Firefox. We have cleared their history and deleted cookies and cached items and the check boxes still do not exist.

I open the report and the checkboxes exist within the prompt page when I open it in Firefox.

It should also be noted that the user can see the checkboxes in IE.
