Creating a web page diaglog for Comments/developer notes

Does anyone know how to create a feedback/developer notes fields on dashboard/report.

I have seem a demo of 3rd party software..Seems it posts comments into the DB so you can then you can report on the comments.

Suggest you look at a product called MBCONTEXT. The product writes comments to a database and they can be reused in other reports etc.
