Problem with AD usernames that have changed

We have a Cognos 10.1 BI implementation that's tied to Active Directory.  I'm having a strange problem when I try to add a certain type of user -- a user whose username has changed. 

The scenario is this -- I'm trying to add an AD user to a Cognos security group.  When a user's name has changed in AD (e.g., Jane Doe was username jdoe, she married and is now Jane Smith, and her username has been changed to jsmith, keeping the same SID), and the user is being added for the first time in Cognos, when I click the "OK" button after finding her in AD and adding her to the group, the action never completes.  By "never completes", I mean the progress bar in the browser moves very slowly, the status bar reads "Waiting for... cognosisapi.dll", then when the bar fills up several minutes later, the screen hasn't changed.  I don't get an error message, and I don't get any entries in the log file (at least none that I've found).  Other AD accounts are added quickly without problem (when adding them separately from one of these problem accounts).

Any ideas?
