Prompt with date and specific time

Hi,  in my prompt page I have to add date and time prompt.

I have to provide the radio button option to the user like below:

a) prior day with 5am to 5pm
b) prior day with 6am to 6pm
c) priorday(no time)

any idea how to accomplish this in the query and prompt page. if anyonw knows please let me know it is very urgent.
