Everything seems quite clear if you are using PVU licensing (I guess ILMT is required: "You must use install and configure reporting tools designated by IBM for reporting usage and provide such reports to IBM upon request in accordance with IBM's guidelines. ").
But what if I all users of the test server already have named user licenses?
Is it simple as "get ec2 instance with OS, install CA, configure security to allow only the named users and start using"?

My take on this is that for named user licenses, you just need to be able to prove that only those users have access, and also that you are adhering to the limitations of each user's license (eg not granting Analytics Explorer capabilities to an Analytics User licensed person).
The safest course of action is to double-check this with your IBM account manager - they will be able to give you a final answer (ie don't go ahead based on the opinion of a muppet).
