Title: Why does my report prompt page go blank when I run the report?

When I run the report the prompt page displays, and then goes blank after one or two seconds. The behavior is very intermittent.  The report was running fine earlier today, although now it keeps going blank at the prompt screen.  You can see the prompt, and then two seconds later it goes white.  This occurs in Chrome and IE 11 - report is Cognos 11.07.

What's interesting is that if you open it in edit mode, and then run it then it works fine.  If you try running it directly then the prompt page goes blank and you can't run it.

Is there anything defined on the prompt page that is unusual? JavaScript maybe?

It's a single prompt with static choices, so it's pretty straightforward.  Those static choices render different pages stored in a conditional block.  We have other reports that are similar in nature that don't seem to be experiencing this issue.
