Hi,
The basic approach is going to be a layout using table cells, sized explicitly. You're probably going to have to use a monospaced font too. The trickiest challenge you face is in converting numbers into words, but thankfully there are some really clever people on Cognoise who have figured this out already and posted up a solution. You need to measure the spacing on the page and use explicitly sized table cells (or blocks) with the relevant padding defined to position the contents exactly where you want them to be. Size and Overflow plus Padding can be defined using various measures - including points (1/72 of an inch) so you should be able to position things exactly where you need on the page if you measure carefully.
