Permission issues - existing users

I have Cognos Express 9.5 with about 75 users. I have one user who can't login and haven't been able to resolve it after reviewing logs and contacting the AD teams.

The user has been using CE for about a year. Suddenly she can't login to our production environment. We've reviewed the logs and there's no issues in Active Directory that we can see. Her group that she belongs to has users that can login. Also, she is able to login to our testing environment. So, there's probably nothing wrong with her active directory account. We also checked if there were duplicate accounts on the network as suggested at the Cognos Support site.

Any ideas? I've been trying to resolve this for a couple of weeks. I even tried removing the role she belonged to and re-adding it to see if that would reset permissions.

Sounds like bad local cache.

Can she log in from another machine that is known to work for someone else? 

If so, then her machine probably has expired/corrupt/bad cache files - clear out local/temp files from IE and user profile directories, reboot and try again

If not and your company does not use Roaming profiles, sounds like server level caching somewhere.
